چکیده

BK virus is a ubiquitous human virus with a peak incidence of primary infection in children 2-5 years of age and a seroprevalence rate of greater than 60-90% among the adult population worldwide. Following primary infection, BK virus preferentially establishes latency within the genitourinary tract and frequently reactivates in the setting of immunosuppression. In renal transplant recipients, BK virus is associated with a range of clinical syndromes including asymptomatic viruria with or without viremia, ureteral stenosis and obstruction, interstitial nephritis, and BK allograft nephropathy (BKN). BKN most commonly presents with an asymptomatic rise in serum creatinine between 2 to 60 months after engraftment (median 9 months). A definitive diagnosis requires an allograft biopsy. Over the last two decades, BKN has been recognized as an important cause of allograft dysfunction and graft loss in kidney transplant recipients. Nonetheless, there is currently no standardized protocol for the management of BK viremia or established BKN. In this article, a brief overview of the literature on the various treatment strategies for BK-associated clinical spectrum is presented followed by the authors’ suggested approach for posttransplant screening and monitoring for BK virus replication. Suggested treatment strategies are also discussed. *Corresponding author: Phuong-Thu Pham, Department of Medicine, Nephrology Division, Kidney and Pancreas Transplant Program, David Geffen School of Medicine at UCLA, Los Angeles, CA 90095, USA, Tel: (310) 794-1757; E-mail: [email protected] Received November 26, 2012; Accepted November 30, 2012; Published December 03, 2012 Citation: Pham PT, Schaenman J, Pham PC (2012) BK Virus Screening and Management Following Kidney Transplantation: An Update. بررسی مولکولی فعالیت مجدد ویروس BK در ادرار بیماران پیوند کلیه

Background & Amis: BK polyomavirus (BKV) is common infection of childhood that persists in kidney epithelium. BKV reactivation characterized by viruria occurs in 35% to 57% of renal allograft recipients. BK virus was detected in serum samples in 6% to 29% of these patients.  BKV-associated nephropathy occurs in as many as 8% of renal allograft recipients and may lead to . This study was carri...

BK Viremia among Iranian Renal Transplant Candidates

Background: Primary infection with BK virus (BKV) is occurred during childhood and usually asymptomatic, but after initial infection, BKV may persist lifelong in the kidney and genitourinary tract. Reactivation may occur in individuals with compromised immunity such as renal transplant recipients. Due to the role of BKV in BK virus-associated nephropathy (BKVAN) and po...

Transient allograft dysfunction from immune reconstitution in a patient with polyoma BK-virus-associated nephropathy.

During the last 10 years, polyoma BK-virus associated nephropathy (PVAN) has emerged as a serious complication in renal transplant recipients [1]. Due to the establishment of an accurate non-invasive screening procedure measuring polyomavirus BK-viraemia, BK-viruria and decoy cells in urine, PVAN can be diagnosed at early stages [2]. This allows for timely therapeutic intervention, which has si...

Prospective study of BK virus infection and nephropathy during the first year after kidney transplantation.

INTRODUCTION The aim of this study was to assess the prevalence and severity of BK virus infection, BK virus nephritis, and related risk factors among kidney transplant recipients. MATERIALS AND METHODS BK viremia during the first year of kidney transplantation was assessed prospectively in 32 successive recipients. BK virus DNA was extracted and determined in all samples by real-time polymer...

Incidence of BK Virus Nephropathy (BKVN) in Renal Transplant Recipients

BACKGROUND BK virus nephropathy (BKVN) is one of the complications of renal transplantation that causes graft loss in renal transplant recipients. OBJECTIVE To determine the incidence of BKVN after renal transplantation in Hasheminejad Hospital, Tehran, Iran. METHODS In this analytical cross-sectional study, we evaluated 31 consecutive kidney transplant recipients (21 men and 10 women) for ...
